I am suffering from urine incontinence where drops of urine come out after i answer the call of nature.how should i deal with this according to islam?do i have to change clothes everytime before prayer?

The principle is that Deen is easy, and God does not want it difficult for His people.
يُرِيدُ اللَّهُ بِكُمُ الْيُسْرَ وَلَا يُرِيدُ بِكُمُ الْعُسْرَ
God desires ease for you and does not want to be harsh with you.
If the drops do not stop after sometimes, and it is a continuous problem, you may ignore washing or changing the clothes every time. You may use pads to keep your clothes clean.

Drops come after within 5 mins of answering the call,and it stops.it happens everytime i answer the call of nature.do i have to change my clothes each time?

This is not something unusual. One should take care of the drops for five minutes. However, a small amount of impurity can be ignored to avoid hardship in matters of worship.
