Doctor's Mistake While Operating On A Patient
If a doctor operates a patient and some mistake happened an year ago which could harm the patient too.. now after an year if doctor has this guilt what should he do? Should he trace back the patient and confess in front of her though don’t know about the consequences or should he was just ask forgiveness from Allah ?

If the patient was not harmed then there is no need to contact the patient. Only asking forgiveness from Allah would suffice and doing charity is also good.
However, if the patient was harmed because of an intentional mistake (not intended to harm the patient) but the mistake was something which a doctor should have avoided but did not becasue of a mistake at his end, then there is a sharia in place to deal with such situation which is shared in the following thread.

At the time there was no harm but don’t know about late complication.. should contact and ask or leave ?

This needs to be assessed by the doctor whether the quantum of this mistake calls for further inquiry or not and if the mistake is a very serious one that could injure or kill the patient, then the doctor should seek further truth on the matter and if the matter is not a serious one and no harm has been reported to the doctor via any other means then there is no obligation to search for this matter further. Either way, at an individual level, the doctor should ask Allah’s forgiveness because this mistake could have resulted in a much bigger loss.
