While adopting a child, is it unislamic for the couple to given him/her their name in terms of parentage on documents..? What is the logic of maintaining the name of biological parents when he has now been adopted?

It is a lie to change the parentage of the child. It leads to disrupt the whole scheme of relationships. The problems of Mehram relations and inheritance are confused.
