If someone is unable to perform obligatory fasting they are supposed to make up their left over fasts after they recover. If the sickness is of permanent nature, then they can pay fidya for their left over fasts. The amount of fidya for one fast is the average value of meals for one day of the person giving the fidya. It can be given to any deserving person regardless of Muslim or not.

There are certain welfare organization who provide food for needy. They provide rates for cost of of food for one person. You can pay there for 60 people. You also need to repent sincerly.
