Quran 50:16 – God Is Closer To Humans Than Their Jugular Vein
وَلَقَدۡ خَلَقۡنَا ٱلۡإِنسَـٰنَ وَنَعۡلَمُ مَا تُوَسۡوِسُ بِهِۦ نَفۡسُهُۥۖ وَنَحۡنُ أَقۡرَبُ إِلَیۡهِ مِنۡ حَبۡلِ ٱلۡوَرِیدِ﴿ ١٦ ﴾
• Dr. Mustafa Khattab:
Indeed, ˹it is˺ We ˹Who˺ created humankind and ˹fully˺ know what their souls whisper to them, and We are closer to them than ˹their˺ jugular vein.

Please see the following translation by Ghamidi sahab rendered into English by Dr. Shehzad Saleem. It is much easier to comprehend:
Quran 50:16
We have created a human being, and We know also whatever thoughts come to his mind, and We are closer to him than even his life-vein.[16]

[16] The implication is that not only his words and deeds but even the latent thoughts which arise in a person’s mind are in God’s knowledge. God’s knowledge and power embraces every aspect of a person; nothing whether apparent or hidden is concealed from Him.

This ‘closer than the jugular vein’ is in a metaphorical sense and is it about Allah? Or is it about a physical presence and refers to the two angels beside everyone?

It’s about God’s knowledge as explained above. God’s existence transcends spacetime so just like it’s not accurate to consider it in physical sense, it’s also not accurate to only consider metaphorically.
