To call someone a sister does not make her your real sister. This is obvious. However, we do have respect towards some women as our sisters and we feel the same.
I do not think that he said it is sinful in either case. Sin is obvious. Instructions to not to sit with a non mehram alone is a preemptive step to avoid indulging in a real sin.
