(1) Belief in GOD (without any taint of shirk).
(2) Belief in the Last Day.
(3) Lead a righteous life.
Regardless of religion, ritual, language, nationality, and books followed, any individual who fulfills these three requirements attains eternal salvation.
Please see verses 2:62, and 5:69.

The following criteria has to be fulfilled to be called a muslim.
Believe in the oneness of God.
Believe in the angels, books, prophets, and the hereafter.
Saying prescribed five time prayers
Keeping Ramzan fast
Doing Hajj if affordable,
Paying zakat if wealthy.
